A thorough, Medicare-funded health check designed specifically for older Australians.
Once you turn 75, you become eligible for a comprehensive health assessment that is fully funded by Medicare. It is a dedicated appointment with our clinic nurse and your GP that looks at your overall health, your medications, your mobility, your mental wellbeing and your support needs, all in one sitting. It is not about finding problems. It is about making sure you have the right support in place to stay as healthy and independent as possible for as long as possible.
What to Expect
A 75+ Health Assessment is a longer appointment than a standard GP visit, so please let our team know when you book that this is what you are coming in for. It helps to bring along a list of all your current medications, including any vitamins or supplements, as well as any recent test results or specialist letters you have.
Our clinic nurse and your GP will work through the assessment in a relaxed and unhurried way. There is no rush and no part of the assessment should feel stressful or confronting. If you would like to bring a family member or carer along for support, you are very welcome to do so.
If any follow-up is needed after your assessment, your GP will explain what is involved and arrange anything necessary before you leave.

A 75+ Health Assessment is a Medicare-funded comprehensive health review available to all patients aged 75 and over. Our clinic nurse and your GP will work through a structured assessment covering your physical health, medications, mental health, memory, mobility, falls risk, social situation and any support needs you may have at home. The goal is to get a complete picture of your health and make sure nothing important is being missed or left unmanaged.

Older patients are often managing multiple medications, and reviewing these regularly is an important part of staying safe and well. As part of your 75+ Health Assessment your GP will go through all of your current medications, check for any interactions or concerns and make sure everything you are taking is still appropriate and necessary.

Falls are one of the most significant health risks for older Australians and can have serious consequences for independence and quality of life. Your GP will assess your falls risk as part of the assessment and discuss any changes to your home environment, exercise or medication that could help reduce that risk.

Your GP will include a brief assessment of your memory and cognitive function as part of the health check. If any concerns arise, your GP will discuss them with you openly and arrange any appropriate follow-up, which might include referral to a specialist or further assessment.

Loneliness, anxiety and depression are common in older adults and often go unaddressed. Your GP will check in on how you are feeling emotionally and socially as part of your assessment and make sure you have access to the right support if needed.

At the end of your assessment your GP will put together a written summary of the findings and a plan for any follow-up care or referrals needed. You will leave with a clear picture of where your health is at and what happens next.

We will discuss with you what your wants and wishes are for your health. We will go through a document called an ‘Advance Care Directive’. This is a document that acts as a guide for health care staff regarding which medical treatments you want should you become unwell.
