New Patient Form

You can book online at either clinic or give us a call. Let our team know you are a new patient when you book so we can make sure everything is set up for you before you arrive.

Bring your Medicare card and your completed registration form if you have not already submitted it. If you have a health summary or medication list from your previous GP, bring that along too. It helps your new GP get up to speed quickly.
