What to Expect

Your first appointment focused on chronic disease management will usually involve a conversation about your condition, your current treatment and how things have been going. Your GP will review any relevant test results, ask about your day to day experience and talk through what support might help.

If a  GP Chronic Condition Management Plan is appropriate, your GP will explain what is involved and what it means for your care. These appointments take a little longer than a standard consultation, so please let us know when you book so we can set aside enough time.

From there, your GP will schedule regular reviews to keep your care on track. The frequency will depend on your condition and your individual needs.
