What to Expect
Your first appointment focused on chronic disease management will usually involve a conversation about your condition, your current treatment and how things have been going. Your GP will review any relevant test results, ask about your day to day experience and talk through what support might help.
If a GP Chronic Condition Management Plan is appropriate, your GP will explain what is involved and what it means for your care. These appointments take a little longer than a standard consultation, so please let us know when you book so we can set aside enough time.
From there, your GP will schedule regular reviews to keep your care on track. The frequency will depend on your condition and your individual needs.

Keeping an eye on how your condition is progressing is one of the most important parts of long-term care. Our GPs schedule regular check-ins to review your health, adjust your treatment if needed, and make sure nothing is being missed.

If you are living with a chronic condition, you may be eligible for a GP Chronic Condition Management Plan. This is a Medicare-funded plan that outlines your health goals and the care you need, and can give you access to rebated allied health services such as physiotherapy, dietetics and podiatry. Your GP will talk you through whether a plan is right for you.

When specialist input is needed, our GPs can arrange referrals and help coordinate your care between your GP and any other health professionals involved in your treatment.

Many chronic conditions require ongoing medication. Our GPs can prescribe, review and adjust your medications over time to make sure they are working well and that any side effects are being managed.
